Registration Health Requirements

9th Graders
Transfer Students

9th Graders

Physical Requirements:

9th graders:

All students who will be 9th graders in 2025-2026 are required to submit a complete Certificate of Child Health Examination Form, dated on or after August 11, 2024, with complete immunization records. This physical will be acceptable for participation in athletics for one year and 30 days from the date of the physical. Students may not participate in interscholastic sports or team practice until they have filed a sports physical with the athletic office. The IHSA sports physical cannot be accepted as a health requirement for Freshmen or transfer student physicals.
A dental exam record must also be submitted, as mandated by the state of Illinois. The exam must have been completed between November 15, 2024 and May 15, 2026.

Any 9th grade student not in compliance with the physical and immunization requirements will be excluded from school beginning October 15th until the correct information is submitted to the nurse’s office. Dental exam forms are due by May 15th, 2026.

Transfer Students

Transfer students (Grades 10, 11, 12):

From an Illinois High School:

Students must present a copy of their 9th grade physical, or more current complete physical, with updated immunizations, at the time of registration. A sports physical does not meet this requirement.

From another state or military base:

Students must present a copy of their high school physical with updated immunizations at the time of registration. If the previous high school did not require a physical, the students have 30 days to complete the Illinois state requirement. The physical must be completed on the Illinois state form. A sports physical does not meet this requirement.
or: If submitting a prior physical, the physical exam must have been completed/dated within 12 months of the first day of school for the new student, and the form submitted must contain all required parts of the Illinois state form. A sports physical does not meet this requirement.
Must complete an Illinois Eye Exam Form obtained from the nurse.

From another country:

The student has 30 days to complete the Illinois state requirements of a physical and updated immunizations. The physical must be completed on the Illinois state form. A sports physical does not meet this requirement.

Vision and Hearing Screenings

Vernon Hills High School provides screenings for vision and hearing according to state regulations. Specifically, within the high school setting:

Students receiving special education services are screened annually.
Transfer students are screened following their enrollment.
Additional students may be screened in response to parent or teacher referral.

If a student doesn't pass their hearing or vision screening, or if other questions arise, a nurse will contact parents/guardians and may recommend further evaluation.

The parent or legal guardian of a student may object to hearing or vision screening tests for their children on religious grounds. If a religious objection is made, a written and signed statement from the parent or legal guardian detailing such objections must be presented to the school nurse.

With any concerns about your student’s vision or hearing or questions about the screening process, please contact the school nursing team.

General Sick Day Guidelines

To help keep everyone healthy, we want to share some important information about when to keep your child home from school and how to help them get better. Please read below for guidelines on what to do if your child is feeling unwell.

Symptoms
People may have different symptoms when they are sick. Do not send your child to school if they have symptoms such as:
• Fever of 100.4°F or higher
• Stomach pain or vomiting
• Sore throat with fever (contact your provider)
• Persistent cough (dry or wet)
• Diarrhea (3 or more events within 24 hours)
• Rash or open/draining sore(s)
• Excessive tiredness
• Lack of appetite
• Headache, body aches, or earache

When can my child return to school?
If your child has been sick, they can come back to school when:
• They feel well for at least 24 hours or more.
• They are fever-free for at least 24 hours without using medication
• There’s been no vomiting or diarrhea for at least 24 hours.
• For strep throat, they’ve been on antibiotics for at least 12 hours
. • For conjunctivitis, they’ve been using appropriate eye drops for at least 24 hours or cleared by a provider.
• For rashes, they have approval from a healthcare provider.

What you can do if you or your child is sick
• Stay home
. • Get plenty of rest.
• Wash hands with soap and water for at least 20 seconds throughout the day.
• Limit touching the eyes, nose, and mouth.
• Cover coughs and sneezes with a tissue or the bend of your arm.
• Don’t share foods, drinks, or personal items (hats, brushes, combs, lip balms, towels, beverages, cups, eating utensils, etc.).
• Please let the school know if your child has been diagnosed with certain conditions like strep throat, chickenpox, scarlet fever, mumps, pertussis, head lice, etc.

What you can do to prevent getting sick
• Stay away from people who are sick.
• Don’t share personal items or food/drinks.
• Wash hands throughout the day.
