As of September 27, 2011 the Child Health Examination Code Part 665 was amended to require proof of receiving a dose of Tdap (Tetanus, Diptheria and Pertussis) vaccine starting school year 2012-2013 for all incoming freshmen.   Please confirm with your physician that  your student has received the required immunization and provide documentation to the nurse’s office.  It is strongly encouraged for all students to  receive a Tdap booster due to the increase in Pertussis (whooping cough).
